Some more extreme metal vocal techniques bypass the vocal cords almost entirely. Y’all should watch the Charismatic Voice videos where they stuck cameras and stuff down Will Ramos’ throat.
Considering the level of oversight and quality control, probably not much. The only way the general public would know is that people would complain very loudly.
